I downloaded formsgeneration-2011-02-06.tar.gz - tar tvf produced:

gzip: stdin: decompression OK, trailing garbage ignored

test_smarty_form.php
test_upload.php
test_upload_progress.php
upload_progress.gif
tar: Child returned status 2
tar: Error is not recoverable: exiting now

I downloaded the zip and it seemed to unpack fine.

The tar archive for download seems fine. Maybe you downloaded a file that got corrupted for some reason. Please try again.
